Lorem Ipsum is simply dummy text of the printing and typesetting industry. Lorem Ipsum has been the industry's standard dummy text ever since the 1500s, when an unknown printer took a galley of type and scrambled it to make a type specimen book. It has survived not only five centuries, but also the leap into electronic typesetting, remaining essentially unchanged. It was popularised in the 1960s with the release of Letraset sheets containing Lorem Ipsum passages, and more recently with desktop publishing software like Aldus PageMaker including versions of Lorem Ipsum.

Immunization Requirements

Parents must provide one of the following immunization requirements for their student(s) to be enrolled and attend school:

  1. Current, up-to-date immunization records; or

  2. A completed and signed exemption form.

Immunization Requirements

All students who attend Oklahoma schools are required to have the following immunizations (or a signed exemption form): 

    • 4 DTP/DTaP/Td
    • 3 Polio
    • 1 MMR
    • 2 Hepatitis A
    • 3 Hepatitis B
    • 1 Varicella (Chicken Pox)
    • 5 DTP/DTaP/Td (Unless the 4th dose was given on or after the 4th birthday) 
    • 4 Polio (Unless the 3rd does was given on or after the 4th birthday) 
    • 2 MMR
    • 2 Hepatitis A
    • 3 Hepatitis B
    • 1 Varicella (Chicken Pox)
    • 1 Tdap
    • Meningococcal vaccine (MCV4)
    • Human papillomavirus (HPV) for females and males
    • Varicella (Chicken Pox)** A second dose at age 4
    • Flu

Exemptions

Oklahoma State Law allows immunization exemptions for medical, religious, or personal reasons. Parents who opt not to immunize their children will be responsible for providing an approved exemption certificate to their child’s school. ALL exemptions expire at the end of the student’s 6th grade year. A new exemption request should be submitted by the guardian to the Oklahoma State Department of Health prior to the child entering 7th grade.
